Providence is calling a Medical Social Worker.
Our Home Health Department is seeking an experienced Medical Social Worker whose
primary responsibilities will include social work evaluation of patient/family
psych/social and environmental needs, as well as implementation of the social
work treatment plan and documentation consistent with insurance guidelines in
their homes or facilities. Work is normally performed in patient/family homes.
Requires considerable travel. There is extensive telephone contact with families
and community resources. Some work performed in an office setting.
Were seeking an individual who performs all duties in a manner that promotes
teamwork and reflects the Mission and Philosophy of our founders, the Sisters of
Providence.
Position Specifics.
This position is located at Providence SoundHomeCare & Hospice in Olympia,
WA. This is a part time, 32 hour per week opportunity working throughout
Thurston County. Schedule may include one weekend day.
REQUIREMENTS
Education: A Masters Degree in Social work from an accredited school of social
work is required.
Licenses/Certifications: Washington State Licensed Independent Clinical Social
Worker (LICSW) or Licensed Advanced Social Worker (LASW) is preferred.
Registered Counselor, LICSW or LASW ? Associate may be considered. Valid
Washington State drivers license along with an insured, reliable vehicle, CPR
certification and Mantoux testing.
Experience: Requires a minimum of one year experience in health care, with a
preference in Home Health/Hospice Care.
