Providence is calling a RN ? Home Care

The Registered Nurse Case Manager is responsible for providing evaluation and
nursing services on an intermittent basis in the patient?s place of residence.
Functions independently and/or in conjunction with an interdisciplinary team
based on the patient?s identified needs. Practice is within the scope defined by
the Washington State Nurse Discipline Practice Act, based on the nursing process
model and concept of community health nursing therapy.

We?re seeking an individual who performs all duties in a manner that promotes

Position Specifics.
This position is located at Providence Hospice & Home Care of Snohomish
County in Everett, WA. Day Shift (1.0)

REQUIREMENTS
Education: Graduate of an accredited school of nursing. Bachelor?s preferred.
Licenses/Certifications: Current Washington State Registered Nursing License.
Current Washington State Driver?s license and have access to daily use of an
insured, reliable vehicle. Current CPR certification.
Experience: At least two years full time experience practicing as a registered
nurse with one year of recent experience (within last three years) clinical
experience in a Medicare certified Homecare or Hospice agency/facility doing
intermittent visits and case management.
