Title: Physical Therapist - Home Health
Location: Alaska-Anchorage
Providence is calling a full time physical therapist for the Home Health at Providence Alaska Medical Center in Anchorage, AK.

In this position, you will:
* Develop, coordinate, maintain, assess, plan, implement, and evaluate physical therapy under the prescription of a physician licensed in the State of Alaska.
* Service in collaboration with physicians, primary partners, and patient/family.
* Document evaluation, plan, care delivered and outcomes according to hospital and department standards.
* Implement care/services that recognize age/diversity specific need/issues of customers served.

Qualifications for this position include:
* Bachelor’s, Masters, or Doctorate degree in Physical Therapy required.
* Current license to practice Physical Therapy in the State of Alaska.
* Experience and credentials in sub-specialty preferred.
* Basic Life Support required upon hire.
