Title: Physical Therapist
Location: Oregon-Beaverton

Providence is calling a full-time Physical Therapist for a day shift position at Providence Home Health Murray Business Center in Beaverton, OR.

In this position, you will:

* Manage and provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician’s plan of treatment, taking into account individual patients special or age related needs.
* Have skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual’s functional level and potential.

Qualifications for this position include:

* Bachelors degree.
* A current State of Oregon Physical Therapy license.
* Basic Life Support (BLS) for Healthcare Provider certification.
* A valid driver’s license, automobile liability insurance and reliable transportation.
* One-year clinical experience in Home Health or healthcare setting or active enrollment in the Providence postgraduate preceptor program.
* In compliance with the Providence Health & Services-OR Human Resources policy #302 Background Checks”, a criminal background check is required for this position, based on assigned duties.
