Title: Patient Care Coordinator - Home Services
Location: Washington-Everett

Providence is calling a Patient Care Coordinator to Providence Hospice and Home Care of Snohomish County in Everett, WA.

We are seeking a licensed Registered Nurse, Physical Therapist, Occupational Therapist, and/or Speech Language Pathologist with the skills and abilities to perform home health staff RN or Rehab role duties.

In this position you will:
* Serve as a communication hub responsible for coordinating the multidisciplinary home health services for the clinical team.
* Act as a liaison between Central Intake, Data Entry, Scheduling, Quality and Clinical Staff.
* Report to the Clinical/Rehab Manager and acts as an adjunct to the Clinical/Rehab Manager in monitoring individual and Clinical Team performance and productivity.
* Provide communication, information and the tools necessary to maintain best practices in all team roles.

Required qualifications for this position include:
* 3 or more years of experience in the Home Health nursing or rehab field

Preferred qualifications for this position include:
* Degree in Nursing, BSN preferred
* Degree in Physical Therapy, Occupational Therapy, Speech Language Pathology
* Current RN, PT, OT, or SLP licensure in the State of Washington
* Previous supervisory experience
* OASIS Certification
* Case Management experience
* Active and valid Washington State Drivers license, auto insurance and reliable vehicle to use for work or equivalent abiltity to travel to patient homes as needed.

We provide skilled nursing and therapy for patients who are recovering from or living with an acute or chronic illness. Our skilled and compassionate team includes nurses, medical social workers, home health aides, chaplains, occupational/physical/speech therapists, medical directors, and volunteers.
