Title: Physical Therapist
Location: Alaska-Kodiak

Providence is calling a full time physical therapist for the Providence Kodiak Island Medical Center in Kodiak, AK.

*
In this position, you will:
* Evaluate patient/residents physical, social, emotional, and cognitive skills as related to his/her prognosis and plan and implement treatment in concert with physician’s orders and in accordance with professional standards.
* Provide direction and guidance to other caregivers to assist them in following through on the therapeutic plan of care.
* Provide physical therapy services in Inpatient, Outpatient, Care Center, and Home Health settings.
* Document all aspects of care and outcomes in a manner that is clear to others and that meets service standards.
* Plan for patient/resident discharge with attention given to home programs, equipment arrangement, and patient/resident family education.
* Provide patient/resident family education in a variety of physical settings and develop community educational or screening programs as needed.
* Assist in promotion of the department through community awareness outreach projects.
* Assess patient’s functional abilities and limitations and makes recommendations for assistive devices as indicated.
* Implement care and services that recognize age and diversity specific needs and issues of customers served.

Qualifications for this position include:
* Bachelor’s degree in Physical Therapy required, Master’s or PhD preferred.
* One year of experience preferred.
* Current license to practice Physical Therapy in the State of Alaska.
* Previous experience is preferred but not required.
* Basic Life Support required.
* Current Alaska driver’s license with clean driving record and proof of vehicle insurance.
