Title: Registered Nurse – MDS Coordinator
Location: Washington-Olympia
Providence is calling a Minimum Data Set (MDS) Coordinator to Providence Mother Joseph Care Center in Olympia, WA .

We are seeking a detail-oriented Registered Nurse (RN) to carefully document clinical assessments of individuals in our skilled nursing facility.

In this position you will:

* Act as an advocate for patients in maintaining choice, independence, privacy, dignity and security.
* Collaborate with the interdisciplinary team to prepare and administer individualized nursing plans, in accordance to individual service contracts.
* Conduct initial and ongoing MDS assessments of all residents to meet state and federal regulatory compliance.
* Have responsibility for providing direct patient care to subacute patients and long term care residents, which may include those with Alzheimer’s and Dementia.

Required qualifications for this position include:
*
Graduate of an accredited school of nursing
*
Current Registered Nurse (RN) licensure in Washington State
* Current Cardiopulmonary Resuscitation (CPR) certificate for Healthcare Providers
*
Three years nursing experience in long term care or subacute environment
*
Knowledge of MDS 3.0, OBRA, RAI and Medicare requirements/standards

Preferred qualifications for this position include:

Bachelor’s Degree in Nursing (BSN)
*
Experience that includes rehabilitation environment and case management
