Title: Physical Therapist
Location: Oregon-Seaside

Providence is calling a part-time Physical Therapist for a dayshift position at Providence Seaside Hospital, Seaside, OR.

In this position, you will:
* Manage and provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities, in a home setting in accordance with a physician’s plan of treatment.
* Take into account individual patient’s special physical or age-related needs.
* Evaluate, treat, and professionally consult to achieve maximum patient/client independence in accordance with the individual’s functional level and potential.
* Perform all duties in a manner that promotes team concept, and reflects the Sisters of Providence mission and philosophy.

Qualifications for this position include:
* Bachelor’s, Master’s or Doctorate Degree from an accredited Physical Therapy Program, required.
* Current Physical Therapy licensure through the State of Oregon, or temporary license if new graduate awaiting licensing exam results, required.
* Current Basic Life Support (BLS) for Healthcare Provider Certification, required.
* Valid driver’s license, automobile liability insurance, and reliable transportation, required.
* In compliance with the Providence Health and Services-OR Human Resources policy #302 Background Checks”, a criminal background check is required for this position, based on assigned duties.
* Minimum of one-year clinical experience in Home Health, or healthcare setting, or active enrollment in the Providence postgraduate preceptor program, required.
