Title: Geriatric Medicine Physician

Providence ElderPlace Seattle has an outstanding opportunity for a Board Certified Family Medicine or Internal Medicine physician with interest in Geriatric Medicine. Full time. Providence ElderPlace Seattle (PEPS) is an innovative PACE (Program of All Inclusive Care for the Elderly) organization. This is an exciting opportunity for a Geriatrician to serve as a primary care provider for a panel of frail elders and collaborate with a group of skilled Geriatricians. As part of our interdisciplinary team, this position allows the physician to practice in a fully integrated, capitated environment where the focus is on quality and prevention. Responsible for direct medical care of PEPS participants, including physical assessment, diagnosis and treatment. As a member of the interdisciplinary team, participates in the development of an integrated plan of care to meet the individual's health and social needs. Follows patients across the continuum of care, admitting to acute, sub-acute and long-term care settings as appropriate, and serves as liaison to the medical community on behalf of PEPS. Board Certification in Internal Medicine or Family Medicine required. Certificate of Added Qualification (CAQ) in Geriatric Medicine preferred. At least one year of experience in chronic care management, as well as experience working in an interdisciplinary team and performing comprehensive geriatric assessments, is required.
