Providence is calling a Discharge Planner to Providence Little Company of Mary Medical Center Torrance in Torrance, CA.

We are seeking a Discharge Planner that will collaborate with the patient/family, multidisciplinary team and physicians, optimizes the process of patient’s progress through the continuum of care, intervening and executing the discharge plan.

In this position you will:

* Work in a collegial manner with physicians and other consultants to implement individualized discharge plans
* Coordinate activities to progress the patient through the continuum of care under the direction of the Case Manager
* Be proactive in assuring the delivery of appropriate, effective, quality, safe discharge in a cost effective manner
* Maintain availability as a resource to facilitate communication among providers and patient/family to remain involved in the discharge plan
* Facilitate system / process improvements to reduce / remove barriers to discharge. Fully participates and promotes focused programs/projects of the Continuum of Care department. Incorporates into daily activity, education of the multidisciplinary team regarding the Case Management Program functions, goals and outcomes.

Required qualifications for this position include:
* 2-5 years experience in hospital discharge planning or social work.
* Current Basic Life Support BLS certification for Healthcare Providers from the American Heart Association
* DRGs, payor systems (managed care, Medi-Cal, Medicare etc.). Basic computer skills required.

Preferred qualifications for this position include:
* BSW or LVN.
* Discharge Planning Certificate
