Title: Registered Nurse Hospice Triage & Visit
Location: California-Torrance

Providence is calling a Registered Nurse to Providence TrinityCare Hospice in Torrance CA.
We are seeking a Hospice Triage and Visit Nurse to work with our TrinityCare Hospice Team.

In this position you will:
* The Hospice Triage Nurse provides skilled professional support and direction to the patient/family/caregiver as prescribed by the physician and Interdisciplinary Group (IDG), agency policies, law, regulation and CHAP standards.
* Provides patient tele-care to premature infants, infants, pediatrics, adolescents, young, middle and older adults with serious/terminal illness.
* May provide care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.
* Be responsible for the delivery of direct care to patients individually and through the assignment and direction of the patient focused care team, utilizing a collaborative patient care management process, meeting established standards of nursing care and integrating other health care team members.
* Responds to patients and families with knowledge of age-specific needs based on the standards of care of unit/department where assigned.

Required Qualifications:
* 1-year of general nursing experience with current experience in hospice,medical, surgical, oncology or critical care.
* Current CA RN license.
* Current BLS sponsored by the American Heart Association for Healthcare Provider.
* Current California licensed driver with an automobile that is in good working order and insured in accordance with state requirements.
* Fluent ability to understand oral and written English directives, including English text.

Preferred Qualifications:
* Nursing experience in hospice, palliative care, or home health.
* CHPN certification is preferred.
