Title: Discharge Planner/Social Worker
Location: Alaska-Valdez
Providence is calling a social worker for the Providence Valdez Medical Center in Valdez, AK.

In this position, you will:
* Work with residents, their families, guardians, physicians, health care professionals and community resources, to provide medically-related social services to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.
* Protect and advocate for the rights of residents and ensure that the quality of care they receive meets all state and federal requirements.
* Provide coordinated discharge planning services focused on continuity of care, resident and family preferences and available resources.
* Develop, implement and monitor activity programs for facility residents.
* Meet individually with residents to evaluate activity desires and abilities of new residents.
* Support and participate in the Eden Alternative environment and Core Vales and Mission of Providence Health and Services.
Qualifications for this position include:
* A Bachelor’s degree in Social Work required.
* Alaska Licensed Baccalaureate Social Work (LBSW) required.
* One year of recent experience in a medical facility or long term care setting is preferred.
* Effective communication skills are key requirements to success in this position. Applicants must demonstrate the ability to articulately express, in writing, social work interventions, resident assessments, plans of care, and place these in the record in a timely manner as required by facility policy.
* Additionally, the social worker must be able to communicate with residents, families, other professionals, and the community, in a positive, professional manner.
* Successful candidates will have the ability to assess psychosocial behavior and medically related resident needs, and develop a meaningful plan of care, appropriate interventions, and effective discharge plans.
